Unicare Community Health Center (UCHC) provides patient care services in response to a wide range of medical care needs for individuals of all ages — regardless of race, color, creed, national origin, gender or disability — six days a week. UCHC’s primary function and purpose is to prevent disease by health promotion, education in activities and primary, secondary and tertiary prevention modalities; to minimize residual physical and mental disabilities; to aid patients in achieving their maximum potential within their capabilities; to accelerate convalescence and reduce the length of the functional recovery and to empower members of the community to help themselves in maintenance of their health. UCHC offers a diverse selection of programs and services to cater to your individualized health needs.

- Physical Exam
- Adult and Pediatric Immunization
- Health Education and Counseling
- Screening for Cancer (Breast, Colon, Prostate, etc.)
- Diagnosis and Treatment for Diseases
- Primary Care for Acute, Episodic Illness
- Management of Chronic Illnesses such as:
- Diabetes
- Hypertension
- Heart Disease
- Asthma
- Allergies, etc…
- Lab Screening and Analysis
- Family Pact – Family Planning (Birth Control Methods)
- CHDP – Physical Exam (School, Sports, and Foster Care)
- Prenatal Care and Postnatal Care
- Ultrasounds
- Dental Care (only offered at UCHC’s Colton, Ontario, Pomona, Los Angeles (741 S. Alvarado), Fontana and Riverside locations)
- Behavioral Health
- Optometry (offered at UCHC’s Colton clinic)
