Unicare Community Health Center’s (UCHC) mission is to be a community health care organization that treats everyone with dignity, respect and cultural sensitivity to help create an environment in which all can prosper. ​

WHO WE ARE

Unicare Community Health Center is a Federally Qualified Health Center with clinics located in Ontario, San Bernardino, Colton, Fontana, Pomona and East Los Angeles. Unicare Community Health Center is a grass root, community-based, 501(c)(3) nonprofit organization whose duty is to serve the unmet health care needs of low-income persons and their families. The goal is to provide the best care for the patients, and to provide effective integration of services to complement plans for delivery of quality patient care. The objective of Unicare Community Health Center is to deliver quality care to the most number of people in an accessible manner at minimum cost, through the use of innovative system and policy. UCHC is designed to provide cost-effective, accessible quality health care to low-income segments of the population for significantly less than its present cost. ​

HISTORY

UCHC was founded in 2013 by a group of community leaders, business associates and health care professionals in response to financial challenges faced by families in Eastern Los Angeles County and Western San Bernardino County communities; which impeded their ability to afford essential health care services and other supports. Unicare Community Health Center identified a small group of operating medical clinics and began the process of transitioning the clinics into licensed community clinics to be operated by the non-profit foundation. The primary purpose of Unicare Community Health Center is to provide high quality, coordinated, comprehensive and culturally competent medical, behavioral, optometry and dental services to the low-income population.
